Making amendments to my report

When you have an occupational health assessment with us, we will always send a copy to you for your approval before releasing this to the referrer or your employer. At this stage you have the opportunity to provide your consent, withdraw your consent, or request amendments to any factual inaccuracies on the report.

From time to time we may have misunderstood an employee, noted something down wrong, or an employee may have inadvertently given us a wrong piece of information at the time. We fully recognise this, and will gladly correct any such information promptly to ensure there are no mistakes within your occupational health report.

However, our clinical staff will not amend opinions, advice or recommendations. This is because our process is an independent, evidence-based assessments that produce non-biased reports that provide work-related health advice to both employer and employee from a qualified occupational health professional.

What constitutes a ‘factual inaccuracy’ can be unclear at times, so we’ve tried to help by putting together some guidance.

My report has a factual error in it

An error such as a wrong date, diagnosis or wrongly noted treatment plan will gladly be amended promptly for you. Please just include in your request a clear note of which section contains this inaccuracy and what it should be amended to.

Example: Mrs Smith visited her GP on the 24th May and has arranged a follow-up appointment on 24th June.

Inaccuracy: The follow up appointment is with a cardiologist, not a GP, and is on 25th June, not 24th June.

We will amend to: Mrs Smith visited her GP on the 24th May and has arranged a further appointment with a cardiologist on 25th June.

I don’t agree with the advice or recommendations of my report

As an occupational health company, we have been asked to provide our opinion on matters such as fitness for work, and include our advice and recommendations. Therefore, this is not something that we can change by request unfortunately.  

In this instance we would suggest that you send an accompanying message to the referrer or your employer detailing anything you would like them to be aware of.

There are details we discussed that are missing from the report

Often our consultations can be quite long in order to understand the full background and reasons why an employee may have been referred for an occupational health assessment. This helps us to gather clinical and non-clinical information and produce a non-biased, evidence-based report.

However, our reports are specifically focussed on health in the workplace, and will not include anything more than a brief summary of the background in order to give a context to an employee’s health. Furthermore, it may not be appropriate for us to document certain details of the consultation to your employer.

In this instance you may wish to send an accompanying message to the referrer or your employer at the same time as releasing the report, detailing anything you would like them to be aware of.

I would like to provide my feedback on the report

We welcome all feedback, and once the matter has been concluded you will be sent a feedback survey asking you for your opinion on our service.

However in relation to your report, we can only amend factual inaccuracies. If there’s anything you would like the referrer or your employer to be aware of, you may wish to send an accompanying message to them at the same time as releasing the report, detailing anything you would like them to be aware of.
