There are just two quick steps to go through – the first is to provide us with some of the administrative details we need to get you setup, and the second is to agree to our service level agreement (don’t worry, we’ve summarised this for you to save time).

SLA

Well this is going well!

By signing up to an account with Smart Clinic, you’re agreeing to our terms within our standard service level agreement. Please download a copy of this if you’d like to read it in full before signing up. For now, here’s a summary of some of the main points:

  • No setup fees, retainers or ongoing charges
  • Pay-as-you-go fees are only charged for any referrals you make, or any services you request from us. The prices for these are published on our website, in our brochure or on the full SLA.
  • Recommendations for any alternative services, or anything that may unexpectedly cause any additional cost will be subject to your approval before we proceed.
  • All the information you require to set us up as a supplier, including our data policy, cyber security policy and letterheaded payment details can be found here.
  • Late cancellations (with less than one Working Day’s notice) or non-attendances to appointments are chargeable. We would seek cost approval from you before any further bookings are made.

Woohoo, we’re all done.

Congratulations, you’re joining the likes of Monzo, Starbucks and webuyanycar by using Smart Clinic as your new occupational health provider.

We need to run a few checks before we setup your account. We usually do this really quickly, but please allow us up to one working day just in case.

Once we’ve completed the checks, we’ll be in touch to confirm your account is ready, and provide you with some more instructions on how to get started. We’ll also send you through your service level agreement for you to save on file.
